How to use it
- Record the task, source version, prompt, tool/version, date, and reviewer. Write “unknown” when a detail cannot be verified.
- Score each criterion from 1 to 5 and add a short evidence note. Do not award a score based only on fluent writing.
- Calculate each weighted contribution as (rating ÷ 5) × weight, then add the contributions for a total out of 100.
- Record missing information, unsupported claims, edits, uncertainty, and relevant conflicts of interest.
- If comparing tools, use the same source, task, criteria, and revision rules as far as possible.
Important limitations
A weighted score is a structured judgment, not an objective measure of intelligence or general quality. Different tasks may require different criteria and weights. For high-stakes material, use qualified review and authoritative references; do not rely on an AI output or this worksheet alone.
